BIOMECHANICS OF BIRTH INJURY TO THE BABY DURING CESAREAN OPERATION

Abstract

Growth of forensic examinations in criminal and civil cases for low-quality obstetric care has been registered. The index of operative accouchement in the country grew by 17.5% in 2001 and in 2012 amounted to 159.1 per 1000 births. The most frequently used by caesarean operation is an operation in the lower uterine segment crosssectional. All classic manipulation of extracting the baby cause the compression of the head as the between subjects with the prevailing contact surface (inner surface of the hands of an obstetrician) and a limited-contact surface (fingers of an obstetrician). The stronger the pressure on the head, the more distortion can be formed by classical brain injury of compression. During emergency cesarean operation with the baby’s head getting stuck in a narrow part of the pelvis is sometimes applied squeezing the baby’s head through the vagina into the uterus, which further deforms the nape joint and local pressure on the parietal bones can lead to their linear fractures. Classic manipulation of obstetrician’s hands when removing the baby’s head during a cesarean operation may cause deformation of the baby’s head and to traumatic spinal injury. To identify the mechanism of injury requires careful analysis of the entire delivery period and comparing it with the neurosonography, MRI, CT, X-ray of the skull and spine. The analysis is based on the physiological characteristics of the baby and newborn.

FEATURES OF DEVELOPMENT OF THE BRAIN OEDEMA IN THE ACUTE PERIOD OF THE SEVERE BRAIN INJURY

Abstract

In recent years in the developed countries growth of number of the brain injuries of varying severity is noted. Relevance of studying of this pathology doesn’t raise doubts in connection with the reasons of a long invalidization of these patients, and also not typicalness in some cases of a current of brain injury, depending on localization of a trauma, time of delivery of health care, age of the patient, etc. The purpose of the research is to study the features of cerebral edema in acute severe traumatic brain injury in different age groups, and comparing clinical data forensic autopsy. The analysis of 56 sick dead in severe traumaticbrain injury consequence from 2005-2010 which were in traumatologic office of medical association of Stavropol region was carried out clinical and forensicautopsy. Patients which middle age exceeded 40 years less often perished from brain oedema probably because at patients of this age of a cell of a brain are less subject to oxygen insufficiency.

FORENSIC MEDICAL DIAGNOSIS OF HIV INFECTION CONSIDERING THE RESULTS OF EPIDEMIOLOGICAL MONITORING

Abstract

The purpose of the study was to identify medical and social characteristics of the HIV-infection and HIV-associated diseases based on the latest scientific publications in this field along with the results of our own epidemiological monitoring. In accordance with the project is implementing now by the Moscow Bureau of Forensic Medical Expertise the diagnosis of HIV infection should be verified considering the data of Moscow Center for Prevention and Control of AIDS concerning the HIV status of dead persons. During the study the correlative dynamic analysis and detailed structure research of mortality from HIV infection for the first half of 2015 have been performed based on the data of the Moscow Bureau of Forensic Medical Expertise. The quantitative indicators reflecting the forensic medical diagnosis of HIV infection as a primary (original) or associated disease with a distribution by gender, age and stage of the disease have been obtained. As a result of the study the recommendations have been developed to make a diagnosis and complete a death certificate in such cases when the HIV infection acts as an associated disease.

PATHOLOGY IN CHRONIC ENCEPHALITIS OPIUM ADDICTION IN THE DIFFERENTIAL DIAGNOSIS RATIO WITH OTHER FOCAL BRAIN LESIONS

Abstract

The analysis of morphological studies of the brain in 33 cases of acute poisoning with opiates in 20 cases of stroke at a young age with a fatal outcome. In the first group drug encephalitis was detected in 23.0% of the observed cases. Studies suggest that, along with morphological signs of encephalopathy (ischemia neuronal proliferation of ependymal cells of the ventricles of the brain, perivascular and pericellular edema, etc.) take place with the formation of neuronophagia microglial nodules at the site of nerve cells lysed. There was a special kind of encephalitis in chronic addiction — ventriculitis histologically manifested plethora subependimarnyh vessels ependymal destruction, infiltration of the ventricular wall and the vascular plexus polymorphonuclear leukocytes. In forensic practice there is a need to differentiate drugs ventriculitis with secondary changes in the ventricles of the brain, which are often accompanied by encephalopathy. When forensic histological examination identified some morphological differences opium encephalitis and focal brain lesions, including vascular origin. Also found that in chronic opiate poisoning characterized by morphological changes in the brain that are not a manifestation of sepsis and associated with primary infection of the brain and can be attached to the drug encephalopathy. Particular attention is paid to specific features, informative for determining the pathological changes of the internal elastic membrane of brain vessels. Identify options tanatogenesis encephalitis drugs, including bacterial, viral and fungal etiology, cerebrovascular disease, and demyelinating diseases.

Abstract

The article is devoted to the expert interpretation of the usage of the definition p. 6.1.2. of the Medical criteria of the determination severity of the harm caused to the human’s health approved by the Order № 194n by the health Ministry of Russia the 24th of April 2008 in the anatomical elements of the fracture of ethmoid bone. The article shows the procedure of the expert analysis in a casus based on the principle of the examination of the topography of the fracture of ethmoid bone regarding to the anatomical architectonics of the bones of the scull’s basis. That feature is laid in the definition of the given paragraph of the medical criteria.
